Please note that if you answer Yes to any of the questions below that you will not be able to attend a class with Abhaya Mixed Martial Arts without further medical clearance:

Abhaya Mixed Martial Arts Club Rules - Please read the below rules and check that you have read and that you understand them and will follow them:
ETIQUETTE
• Uniforms must be kept clean and sanitized.
• If you get blood, dirt or excessive amounts of sweat on the mats clean it up.
• Should you arrive late, quickly line up at the edge of the mat and request permission to enter class. The decision to allow a late student onto the mat is entirely at the instructor’s discretion. The instructor may prefer the student to watch rather than risk injury due to missed warm ups.
• The mat is more than a place for merely training; it is a place where one seeks to perfect character, and to master their mind, body and spirit. The mat is considered a very special place and is treated with respect.
• No shoes are to be worn on the mat with exception to indoor only boxing and wrestling shoes during applicable classes only.
• Shoes must be worn when upstairs at Abhaya and when on the exit side of the desk.
• There is to be no unnecessary talking when a class is in session, even if you are not in class.
• If it is necessary that you leave class early, let the instructor know before class begins. Otherwise, students are expected to stay on the mats until the end of the training session and bow out with the class. Likewise, even if one is resting between rounds, students should remain on the mats with the class and not wander around the gym.
REQUIRED APPAREL
• Students are expected to attend all classes in a clean gi or rash guard/t-shirt and shorts. There are no sleeveless shirts allowed in submission grappling.
• For no-gi jiu-jitsu students are expected to wear proper grappling shorts. No pockets, zippers or other metal parts allowed. This is for your partners safety as well as protecting the mats.
• Students are required to keep their gi in good order. There can be no holes in the gi as this can be a safety hazard.
• Students attending boxing or kickboxing are expected to purchase and maintain their own boxing gloves and shin pads.
PERSONAL HYGIENE
• Students are expected to maintain their personal hygiene. This includes trimming nails, tying back long hair and being mindful of body or breath odour. This should be given appropriate attention out of both respect to other students and prevention of the spread of diseases.
• Diseases such as staph infection and ringworm can be transmitted through skin-on-skin contact. Keeping yourself and your apparel clean can significantly reduce the chances of infection. Students that attend class with an unclean gi, rash guard etc. will be asked to leave the mat.

Waiver and Release *
To the best of my knowledge, I am in good physical condition and fully able to participate in any and all Abhaya Mixed Martial Arts programs/events/classes/training sessions. I am fully aware of the risks and hazards connected with the participation in Brazilian Jiu-Jitsu, Judo, Submission Grappling, Kickboxing, Boxing, Mixed Martial Arts, Fitness Training, Weightlifting, or Aerobics, including physical injury or even death, and hereby elect to voluntarily participate in said event, knowing that the associated physical activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or loss or damage to property owned by me, as a result of participation in this course. I further certify that I am at least 18 years of age. If under 18, my parent/guardian is the below signed.
I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, Abhaya Mixed Martial Arts, 3307851 Nova Scotia Limited, Jonathan Foster, their officers, servants, agents, operators, directors, agents, sponsors, instructors, employees, independent contractors, volunteers and members (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me as a result of any “Abhaya Mixed Martial Arts”, physical or athletic activity, physical instruction or sport conducted or carried on by or for “Abhaya Mixed Martial Arts”, whether by itself or with others or in or occurring while he/she is on any premises or property occupied or used by “Abhaya Mixed Martial Arts” or at any “Abhaya Mixed Martial Arts” event, whether caused by active or passive negligence, fault, failure or otherwise howsoever.
It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE, DISCHARGE, and CONVENTION TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in accordance with the laws of the Province of Nova Scotia.
Any photographs or motion pictures taken during participation at "Abhaya Mixed Martial Arts” may be used for promotional purposes. The "Member" also acknowledges that "Abhaya Mixed Martial Arts" is the sole owner of all pictures and videos recorded on "Abhaya Mixed Martial Arts" premises and any related events.
The "Member" further agrees to release "Abhaya Mixed Martial Arts" its instructors, representatives and members from any liability for any damage, loss or theft of personal property.
The "Member" understands that during the course of instruction, employees of "Abhaya Mixed Martial Arts" and/or other students or authorized persons will be engaged in a course of conduct requiring physical contact, and the "Member" gives full consent to such contact as is required by the training. The "Member" signs this waiver of liability fully realizing that his/her participation or engagement in the activities of "Abhaya Mixed Martial Arts" may subject him/her to personal injury or bodily harm.
The "Member" confirms that engaging in martial arts is a potentially hazardous activity and may result in injuries to him/her for which he/she accepts full responsibility.
In signing this release, I acknowledge and represent that I HAVE READ THE FOREGOING Waiver of Liability, that I have answered all questions in the Physical Activity Readiness Questionnaire truthfully and to the best of my knowledge and Hold Harmless Agreement, UNDERSTAND IT AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreements have been made; and I EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME.
